To help familiarize new patients with our office, we have created this page to help you understand our office policies. If you have a question that is not answered on this page, we encourage you to contact our office and we’ll be glad to help you.
For the convenience of our patients, we offer online registration. Click here to visit our patient portal to register online. If you prefer, you can also download the patient registration forms to the right of this page and bring those in. When you come in to our office, please bring a valid driver’s license or state issued ID along with your insurance card (if you have insurance).
We are open Monday – Saturday from 8am – 8pm.
Southeast Medical Center accepts most insurance plans. Please be aware that not all physicians of Southeast Medical Center participate in all plans. If you have any questions regarding insurance please contact our office.
Southeast Medical Center will file your claim for you, however any co-pays, deductible, or services not covered must be paid in full at the time of service.
Most prescription refills require an office visit for physical evaluation. Exceptions will be made on an individual basis, for current patients of Southeast Medical Center who run out of anti-hypertensive, or anti-diabetic medications, etc.
When calling our office to request a refill, please have the name of your medication, pharmacy name and phone number available to give to us. Please allow up to 48 hours for refills to be approved and sent to your pharmacy.
We accept cash, credit and debit cards. We do not accept checks at this time.
We see patients by appointment and we do accept walk-ins for urgent illness or injury as needed. We also offer weight loss regimens to fit your lifestyle goals. We request that you notify our office if you need to cancel your appointment within 24 hours of your scheduled appointment time. No-show appointments may be charged $25.
